Haemodynamic and electrocardiographic effects of frusemide during supine exercise in patients with angina pectoris.

چکیده
The influence offrusemide on haemodynamic, electrocardiographic, and symptomatic response to exercise was studied in 19 patients with documented coronary artery disease. During the control exercise period all patients experienced angina and showed ST segment depression, accompanied by raised pulmonary artery wedge pressures (PWP). In 12 of these patients 40 mg frusemide was administered intravenously and in the other seven patients 40 mg frusemide was given orally. After significant diuresis (>400 ml), the exercise test was repeated. Data were collected at the same exercise work load and duration as had been achieved before frusemide. Both in the intravenous and oral group there was a reduction in cardiac output at rest and a less obvious reduction during exercise. The PWP during exercise after intravenous frusemide decreased from 32-9 to 11P8 mmHg and after oral frusemide from 30 7 to 15-7 mmHg. During the control period there was evidence of mitral regurgitation in seven of the patients during angina, as shown by high amplitude v waves in the PWP recordings. These alterations disappeared completely after the administration of frusemide. Ischaemic ST depression improved in 15 of 19 patients after frusemide (0-21 vs 0 09 mV). Furthermore, there was a reduction of complete suppression of anginal pain in 18 of 19 patients. Ten patients were capable of continuing exercise for a further period of time at the same work load. These data, demonstrating the antianginal properties of frusemide, may be explained by the reduction in ventricular volumes and pressures, resulting in a decrease in myocardial wall stress and oxygen demand. The effects may be the result ofboth the diuretic action offrusemide and its venodilator capacity.

Comparison of Adrenergic Beta-blocking
The symptomatic, electrocardiographic, and haemodynamic effects of two adrenergic beta-blocking drugs, oxprenolol and propranolol, have been compared in equipotent intravenous doses in six patients with uncomplicated angina pectoris during treadmill exercise. The method of comparison included double-blind assessment and analysis with placebo control and randomized serial comparison in each pati...
متن کاملCentral haemodynamics during oxygen breathing in angina pectoris.
Lecerof, H. (1974). Thorax, 29, 673-677. Central haemodynamics during oxygen breathing in angina pectoris. The haemodynamic effects of breathing 100% oxygen have been studied by right heart catheterization at rest and during bicycle exercise in nine male patients with coronary heart disease. Oxygen breathing produced no significant haemodynamic benefit during angina pectoris induced by physical...
متن کاملMyocardial ischaemia and ventricular arrhythmias precipitated by physiological concentrations of adrenaline in patients with coronary heart disease.
The clinical and haemodynamic effects of adrenaline infusion (30 ng kg-1 min-1) producing plasma adrenaline concentrations in the range seen during acute myocardial infarction and of placebo were investigated in a crossover design in 14 patients with stable coronary heart disease. Adrenaline infusion resulted in electrocardiographic evidence of myocardial ischaemia (greater than or equal to 1 m...
متن کاملHaemodynamic comparisons of atrial pacing and exercise in patients with angina pectoris.
Though exercise is the most physiological test for angina pectoris, it has several disadvantages. The most important are that, it is difficult to achieve a steady state, comparable haemodynamic parameters are not readily reproducible during repeated periods of exercise (Burkart, Barold, and Sowton, 1967), it cannot be used effectively in patients with poor exercise tolerance, and angina pectori...
متن کاملClinical, electrocardiographic, and haemodynamic effects of digitalis (ouabain) in angina pectoris.
The symptomatic, electrocardiographic, and haenodynamic effects of acute digitalization with ouabain were studied in 6 male patients with uncomplicated angina pectoris. Ouabain improved the depressed left ventricular function associated with the onset of pain in all patients, without aggravation of their symptoms or further deterioration in the electrocardiogram. It is suggested that the reduct...
